I solemnly swear that I was born at Copenhagen, Denmark, on or about the 19th day of February, 1888, that my husband, Charles Jensen was born in Denmark, that his father emigrated to the United States, sailing on board the from Liverpool on or about 1887; that he resided 31 1/2 years, uninterruptedly, in the United States, from 1887 to 1919 at Chicago, Ill; that he was naturalized as a citizen of the United States before the Circuit Court of Cook County at Chicago, Ill, on Oct 12, 1892 as shown by the accompanying Certificate of Naturalization; that I am the daughter-in-law of the person described in said Certificate; that I have resided in the United States, uninterruptedly, for 11 1/2 years, from 1907 to 1919, at Chicago, Ill; that I am domiciled in the United States, my permanent residence being at 4436 Springfield Avenue, Chicago, in the State of Illinois, where I follow the occupation of housewife. My last passport was obtained from _never had one_. I am about to go abroad temporarily; and intend to return to the United States within 6 months with the purpose of residing and performing the duties of citizenship therein; and that I desire a passport for use in visiting the countries hereinafter named for the following purpose: Denmark - Restoration of health. I intend to leave the United States from the port of New York sailing on board the about April 15, 1919. OATH OF ALLEGIANCE. Further, I do solemnly swear that I will support and defend the Constitution of the United States against all enemies, foreign and domestic; that I will bear true faith and allegiance to the same; and that I take this obligation freely, without any mental reservation or purpose of evasion: So help me God.
